   officers and directors faultless in the event of injury or other harm occurring to the team’s players during
   the participation in all tournament events. By signing this “Waiver of Liability” the coach attest to the
   tournament organizer that adequate medical insurance is available and if necessary will be responsible for
   any medical expenses.

   Full Payment confirms teams slot in tournament. No personal checks. Make cashiers check or money
   order payable to “Metro Sixers”. Mail payment along with completed registration and roster form to:
   Metro Sixers, P.O. Box 2025, Bowie, MD 20718. Note: No refunds will be issued for cancelled
   registration. If you are registering for a tournament all teams must be prepared to play first game by
   6:00 PM on Friday night. Please plan your travel schedule ACCORDINGLY.
